Index finger, or whichever finger you use to push the needle through.

Do you know if they make ones for thumbs? Or ones that get large enought to fit on thumbs?

Yes they do but I think they are harder to find. I've only seen really big ones made out of some-what soft plastic (still hard enough to keep the head of a needle from going through)

I always put it on my middle finger, although I know plenty of folks who use the index finger. I think it depends on what you're comfortable with.

Okay! Do you use anything to keep the needle from slipping? I have the bad habit of biting my fingers, and so if it hurts and I can't get a super good grip, I can't hold onto the needle. Do you think that something like a rubber finger would work for that?

I don't use anything, but if it's tough to pull through I use pliers rather than hurting my hands. I think my mom taught me that.

Rubber finger might work? I think you should try it and see if it works for you. And if it doesn't, try something else. There isn't a definite "do this" way as long as you're getting the stitches in where you want them.

I actually bought a leather thimble with a metal piece on the fingerpad that might work for a thumb.
